Destination Medicine Coordinator
Job Summary
Responsible for acting as liaison between patients, families, caregivers, and various community–based organizations to ensure that the program consistently provides optimal care and service experiences that surpass expectations. As the designated patient liaison for the Destination Medicine Program, coordinates health care services and the patient care experience for contracted Destination Medicine agreement patient referrals and for patient self-referrals for health services. Responsible for logistics including, but not limited to travel, accommodations, language translation, and concierge services.
Job Duties
- Utilizes customer service skills to determine and manage necessary actions to facilitate patient navigation through the Program and acquisition of health services.
- Provides assistance with scheduling, pre-arrival information, transportation, lodging, and special needs for both the patient and any travel companions.
- Provides information on, and assists with access to, local community services and activities that are available during their stay.
- Acts as a key resource and primary point of communication for all patient and travel companion needs.
- Maintains systems and processes sufficient to assure timely communication, service, and follow-up.
- Escorts patient to all appointments while a patient and ensures all questions to the appropriate Service Line are quickly addressed and answered.
- Serves as the designated patient representative and concierge for any needs which may arise during the entire stay.
- Arranges for risk screening for all first-order relatives sharing the same genetic risk who accompany the patient.
- Assure that patients have access to records
