RN - Registered Nurse Case Manager/PI Coordinator Trauma

Job Duties
The Registered Nurse Case Manager/PI Coordinator Trauma maintains and updates the trauma registry and performance improvement database. Assists with the development and implementation of the trauma performance improvement program.
This role is full-time, 40 hours weekly; Monday through Friday; typical hours are 7:00 a.m.- 3:30 p.m.
A minimum of 3 years RN work experience is required. Previous Trauma experience is strongly preferred.

Job Duties:
- Conducts daily clinical rounds on trauma patients providing clinical and systems focused assessment.
- Concurrently obtains demographic, clinical and outcomes focused information from trauma patient medical records and electronic database sources.
- Assures a high degree of accuracy for all data coded, abstracted and entered into the database.
- Prepares and obtains accurate trauma patient information and reports concurrently within 6 weeks of discharge.
- Collaboratively assures compliance with regulatory standards.
- Facilitates the development of action plans related to quality issues and implements resolution of system issues in collaboration with leadership.
- Maintains, updates and enters data into the system collaboratively with team members and provides reports to support performance improvement initiatives.
- Collaboratively develops daily patient log and prepares agendas.
- Serves as a liaison and collaborates with all departments involved in the care of the trauma patient.
- Assists departments as needed in trauma data management.
- Communicates with referring institutions and providers and reports all quality improvement issues and discharge status.

Graduate from Specialty Training Program-Nursing (Required), Bachelor's Degree-Nursing (Preferred)
